It is one of the most asked questions about shipping: do containers really fall off ships? The answer is yes — but far less often than legend suggests, and the industry has been driving the number down. Here is what actually happens when a box meets the ocean.

How often it happens

Out of the hundreds of millions of containers shipped each year, industry surveys count the boxes lost at sea in the low thousands in a bad year — and in recent years, often below that. Losses are rare per voyage, but concentrated: a single severe incident in heavy weather can account for most of a year's total, when entire stacks collapse at once.

Why containers go over

Cargo owners claim on marine insurance; ship operators answer to flag and coastal states; and in major casualties, the ancient principle of general average can apply — spreading extraordinary losses across all cargo interests on the voyage. A single stack collapse can generate claims worth many millions.

The response has been systematic: mandatory weight verification, smarter stowage software modeling stack forces, route planning that avoids the worst seas, updated lashing standards, and mandatory reporting of losses so the data drives improvement. Some operators now receive real-time warnings when their ship approaches conditions favorable to dangerous rolling.
